概要 Objective: To identify factors associated with home return from Japanese geriatric health services facilities (GHSFs) and determine clinically informative cutoff values.
Methods: This retrospective cohort study included 215 residents discharged from a GHSF between 2021 and 2024. Age, Barthel Index (BI), cognitive function, cohabiting family members, care level, and grip strength were extracted from medical records. Factors associated with home return were examined using multivariable logistic regression, and cutoff values were determined by receiver operating characteristic analysis.
Results: Younger age (odds ratio [OR], 0.895; 95% confidence interval [CI], 0.850–0.942), higher BI (OR, 1.052; 95% CI, 1.034–1.071), and cohabiting family members (OR, 7.51; 95% CI, 2.99–18.83) were independently associated with home return. The area under the curve was 0.767 for BI and 0.681 for age. Cutoff values were 62.5 for BI and 87.5 years for age.
Conclusions: Age, ADL independence, and cohabiting family members were associated with home return from Japanese GHSFs. The cutoff values may support risk stratification and home-return planning when considered alongside other clinical and environmental factors.
